Technote Number: 1104996

Problem:
This issue was reported to Lotus Software Quality Engineering and has been
addressed in the 6.0.x codestream in Notes Domino 6.0.2 CF2. The issue has not
been fixed in Notes Dominio 6.5 or 6.5.1

Excerpt from the Lotus Notes and Domino fix list
(http://www.lotus.com/ldd/r5fixlist.nsf):

Technical Area: LotusScript
SPR# BMGH5HKPV5 - Fixed a problem where a Form with Header was preventing
LotusScript procedures defined in external objects from loading.

Workarounds:

In the Declarations event for the button (or design element which is not seeing
the library) add a USE statement to call the library.

Alternatively you could remove the Form Header. Edit the Form and place the
cursor at the top of the form. Select Design, Form Properties and select the
fifth tab and then uncheck the "Add header to form" property.
